Before your visit

• Call ahead to schedule. New patient visits work best with an appointment.
• Arrive at least 15 minutes early so we can get you checked in.
• Bring your photo ID and your insurance card.
• Bring a list of your current medications.
• Print and complete the New Patient Request Form if you can.

What to expect

Our staff will give you a short, single page information form to complete before you are seen. Dr. Crider will take time to review your history, talk through your concerns, and lay out a plan that fits you.

Most major insurance plans are accepted. If you are unsure about your coverage, call us and we will help you sort it out.
